Licensing Program Analysts (LPAs) T. Tran and A. Carter arrived at the above facility to conduct an unannounced Case Management Incident inspection that was self-reported on 4/25/24. Monterey Park SW Regional Office received the writing incident report on 05/13/4. LPAs met with Site Supervisor, Monica Guerrero and Leslie Gamero, Education Manager.
