What the inspector wrote

On 10/23/2025 Licensing Program Analyst Mariah Aguirre conducted an unannounced Annual/Random inspection to the above facility. Upon arrival, LPA disclosed the purpose of the inspection and met with Licensee Xiao Guan who guided the LPA on a tour of the facility. Licensee speaks english, but prefers Mandarin. LPA called Youlie Qi, support staff to translate for LPA. Adults in the home were discussed and criminal record clearance was obtained by those living in the home. Per licensee, there are four adults living in the home. Licensee states that there are currently 12 children enrolled. At the time of inspection, 9 children were present in the home with licensee, licensee's husband, and one staff member. The children's roster was reviewed and is current. Per licensee, the facility\342\200\231s hours of operation are Monday to Saturday 8:30 AM- 6:00 PM. Emergency Disaster Plan, License, and Parents\342\200\231 Rights were posted on parent board at the time of inspection. Disaster drill log was also available during today\342\200\231s inspection, last disaster drill conducted was on 10/14/2025. LIC 126 was provided to licensee in Mandarin. This is a two-story home which consists of 4 bedrooms, 4 bathrooms, kitchen, dining area, living room, detached garage, front yard and backyard (fenced). The children use the living room and downstairs bathroom. Per licensee, the children do not have access to either the front or back yards. Per licensee, areas off limits to children and parents include: the entire second floor, including all 4 bedrooms, 3 bathrooms, kitchen, and detached garage. All areas identified on the facility sketch were inspected. The licensee provides food for children in care. Licensee uses cellphone as a working telephone in the home and is in contact with parents via call or text. LPA observed age-appropriate toys, clean, free of loose and sharp parts. LPA observed electrical cover outlets all through the areas accessible to children. Detergents, cleaning compounds, medications, and other items which can pose a danger to children are inaccessible to children kept under the kitchen sink. Page 1 of 4 The licensee states that 4 adults and 0 minors currently live in the home. Licensee states that they currently have one assistant. All individuals present in the home have obtained a criminal record clearance or exemption prior to working, residing or volunteering in a licensed home. Per licensee, there are no weapons, firearms or bodies of water on the premises. LPA was led on a tour of the facility by the licensee. LPA observed 9 children present during inspection. There were age appropriate toys and equipment present for children in care to use. Emergency Disaster Plan, Parent\342\200\231s Rights Poster and the Facility License are observed to be posted. Per licensee, a corner of the living room will be used as an area for an ill child until an authorized representative comes to pick up child. Fireplaces and open face heaters are inaccessible to prevent access by children. There was a safety gate that leads to the hallway and kitchen that makes the staircase in accessible to children in care. The bathroom children use is directly accessible from the living room. Detergents, cleaning compounds, medications, and other items which could pose a danger are inaccessible to children by safety latch. There are first aid supplies available. Per licensee, they provide all infants with formula. Upon observation, licensee stores the formula in a cabinet in the kitchen. Per licensee, they provide breakfast, lunch, and afternoon snack to children in care. They are part of the Options for Learning food program. Licensee provides all the food to children in care. Napping equipment for children in care was observed, all children in care sleep on napping cots observed to be stored in the downstairs bathroom, and napping equipment such as blankets, are provided by parents and are kept in a downstairs closet for each child. The valve on the required 2A10BC fire extinguisher was newly purchased on 07/19/2025 and is operable. Smoke and carbon monoxide detectors are in operable condition. Upon observation, first aid kit is fully equipped with required first aid supplies. Page 2 of 4 The Licensee has current Pediatric First Aid and CPR. Upon Staff file review, Licensee\342\200\231s file, licensee's husbands file, and one staff file was incomplete with required forms and documents. One staff member has not completed Mandated Reporter training along with having an incomplete file, and has incomplete immunizations. As a result, three type B citation was issued (see attached LIC 809D). Upon children\342\200\231s file review, all children\342\200\231s files were reviewed and complete. Infant sleep log was observed to be complete and LPA reminded licensee that all infants in care must obtain a 15 minute sleep log. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers and any other item that falls into that category are permitted in the facility. Effective January 1, 2010, licensees of family childcare homes are required to ensure that at least one staff member with current training in pediatric first aid and pediatric CPR is on site at all times when children are present. AB 1207 : Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. LPA discussed the influenza waiver during the visit. LPA reviewed and issued the LIC 311D - Forms/Records to Keep in Your Family Child Care Home. Mandatory Forms for the children\342\200\231s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Licensee was advised how to access forms and Regulations online at www.ccld.ca.gov . Licensee was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care. Page 3 of 4 Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee, and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. MyChildCarePlan.org \342\200\223 Centers and Family Child Care Homes Licensee, was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the licensee confirmed that there are no Registered Sex Offenders living in the home and LPAs completed the RSO profile in FAS. Exit interview was conducted and a copy of the report was provided to Licensee, Xiao Guan along with Appeal Rights. Page 4 of 4
