On 10/27/2025 at 10:25AM Licensing Program Analyst (LPA) Mariah Aguirre conducted an unannounced Case Management POC inspection to review staff files that the facility was cited three type B deficiency for on 10/23/2025. Upon arriving to the facility, LPA was greeted by Licensee Xiao Guan, and licensee's husband Wei Dong Liu. LPA reviewed the staff files and previously missing paperwork to be readily available and complete. At this time, deficiencies previously issued are being cleared. An exit interview was conducted with Licensee Xiao Guan. A copy of this report along with appeal rights, proof of correction letters, and a Notice of Site Visit were issued during the exit interview.
