Inspector’s narrative

What the inspector wrote

While conducting an investigation for a complaint, Licensing Program Analysts (LPAs), T. Tran and A. Carter observed the following deficiency. During the interview conducted with licensee, facility had three cases of head lice occurred on 5/22/24 and licensee failed to report this incident to the licensing department. LPAs discussed the importance of reporting requirement. LPAs obtained the declaration statement that licensee acknowledging the importance of reporting requirement moving forward. Facility was cited a type B deficiency and cleared during today's visit. See Facility Evaluation Report LIC 809D for deficiency cited. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Francis Tomas.

The licensee did not comply with the section cited above requirement of: Immunizations prior to admission to a family day care home, children shall be immunized against diseases as required by the California Code of Regulations , Title 17 to which poses a potential health, safety or personal rights risk to persons in care.
