What the inspector wrote

Licensing Program Analysts (LPAs) Jeanette Estrada and Christine Nolan conducted an unannounced annual inspection at the facility. LPAs met with Staff 1 - Irma Arredondo. Per Facility Representative, the Director and Assistant Director are out for the day. LPA advised to have form LIC 308 Designation of Facility Responsibility reflect any updates. This is a preschool program which operates Monday through Friday 7:00 AM to 5:30 PM. This program also offers a half day schedule if needed. The program provides care to children ages 2-5 years old. This program is located across the street from 3801 Cortland St. Lynwood 90262. There is a second floor connected to this facility. LPAs observed the stairs to be blocked and inaccessible to this facility. This facility operates in 3 classrooms. LPAs inspected areas available to children in care. Required posted documents were observed in the main office and in the classrooms. There are age appropriate materials, furniture and equipment in the classrooms which is in good condition. Cleaning products are kept in cabinets out of reach of children. Classroom 1 and 2 share two restrooms which contain 2 toilets and 2 sinks each. Classroom 3 has it's own restroom with 1 toilet and 1 sink and there is a diaper changing table in the classroom as the program assists with toilet training. Medication is administered with a prescription. Medication is kept in the office. LPAs observed an operable carbon monoxide detector in the kitchen. There is air conditioning in each classroom. Drinking water is provided via filtered water connected to the kitchen. The facility tested for lead in drinking water on 8/16/23 with no lead exceedances detected. The facility provides breakfast, two lunch meals and a late afternoon snack. Meals are prepared on site in the kitchen. LPAs observed kitchen to be kept clean and organized. Menus are posted once a month for parents to review. Children with allergies are provided an accommodation. Page 2 Each classroom has a designated enclosed outdoor area. There are toys and materials available in good condition. There is also a separate enclosed yard in the parking lot which is designated for the facility. There is shade available and cushioning under the play structure. The last emergency drill was conducted 10/14/25. The isolation area is located in the office. LPAs reviewed children's records for completeness. Staff records were reviewed. At least one staff has a valid pediatric first aid/cpr certification. 4 out of 5 staff did not have a complete record of required immunizations and 1 out 5 did not have a completed LIC 503 with TB clearance. Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP . Two type B deficiencies were cited during today's inspection -See LIC 809D A notice of site visit was given to Facility Representative and must remain posted for 30 days. Exit interview conducted and report was reviewed with Facility Representative, Irma Arredondo.
