What the inspector wrote

On 11/18/2025 at 9:00AM Licensing Program Analyst (LPA) J.Duran conducted a Required Annual Inspection at the above facility. Upon arrival, LPA met with Licensee Rita Serna. There were nine (9) children present including one (1) infant. LPA explained the purpose of the inspection and toured the facility. LPA inspected rooms/areas on the facility sketch in which child-care services are provided and to which children have access. Per licensee the hours of operation are Monday-Friday 6:00am-6:00pm. All adults present have received fingerprint clearance. Individuals residing in the home were discussed and noted. This is a single story home located on a lot with an additional dwelling that has a different address. The childcare home consists of 2 bedroom/2 bath. The main childcare areas are the child care room/den in the back of home, the covered patio and backyard. The living room in the front of the home is used as nap room. Infant cribs and mats were observed. The children use the bathroom in the hall by the kitchen. Off limit areas, per Licensee and facility sketch are: 2 bedrooms, 1 bath in main bedroom. Off limit areas were kept locked and inaccessible to children in care. LPA observed a fire place in the childcare/den to be permanently covered with \302\275 inch x 5 inch boards making it inaccessible to children in care. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. LPA observed required posted documentation in facility entrance which included: Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. LPA observed completed facility records including; LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan. Fire extinguisher indicated fully charged and was serviced on 02/25/2025, Licensee was reminded that fire extinguisher needs to be serviced yearly. Page 1 of 4. *****Inspection was conducted in Spanish***** Children\342\200\231s records (6) were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents\342\200\231 Rights, and LIC9150 Additional Children in Care for Large FCC all were in children's files. Staff records were reviewed: Licensee Pediatric First Aid and CPR certification 02/07/2027, Mandated Reporter exp 02/13/2027. One staff had no Mandated Reporter on file which poses a potential health, safety or personal rights risk to persons in care, type B deficiency cited. Per licensee they were in the process of completing the training no later than 12/05/2026. Smoke and carbon monoxide detectors were tested and are operable. The home maintains telephone service via cell phone/LAN line. The home is observed to be clean and orderly. There are toys and other age-appropriate material available. LPA observed that cleaning compounds are in kitchen and bathroom inaccessible to children in care. LPA observed cleaners and Insect spray in locked cabinet in hallway. The bathroom that children use is located in the hallway and observed to be clean and free of hazards. Per Licensee there are no firearms or weapons stored in the home. Isolation area for sick children waiting to be picked up is in one of the day-care rooms, where children are supervised and away from the other children. Currently, children are using the enclosed patio and backyard for outdoor play. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with including a two level play structure and swing set, which is anchored to the ground with natural grass under structure that cushions a fall. LPA did not observe any objects that could be hazardous to children in care. The facility does not have a pool or similar bodies of water. One type B deficiency was cited during todays inspection. Licensee Rita Serna was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee Rita Serna and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee Rita Serna was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Page 3 of 4. During the exit interview, the Licensee Rita Serna, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
