What the inspector wrote

Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced Case Management inspection for the purpose of following up on an incident reported to the Department on 03/05/2025. Upon arrival at 9:45am, LPA met with Lead Teacher Liza Cantabrana, to whom the purpose of the inspection was explained. A tour of the facility was provided. There were children present during the time of inspection. Census was taken. There were 19 children with 4 staff members. On 03/05/2025, a child sustained an injury while outside on the play structure, that resulted in the injury needing additional medical evaluation. Child #1 was climbing up the structure ladder when they reportedly slipped on the spiral ladder, resulting in a cut to child. The purpose of today's inspection was to ensure proper supervision was provided and that the facility equipment is being maintained in a good condition. During today's inspection, LPA conducted interviews with two staff members and one child. Documentation in the form of facility injury report were obtained during inspection. During interview, Staff #2 stated they were next to child the entire time and were able to immediately assist the child when they got hurt, helping push the the child up and pick them up by the waist to reach the top. Child #1 disclosed that they Staff #2 helped them up to the top when they got hurt. LPA reviewed the play structure area. The structure was observed to be age appropriate for the children in care, and did not appear to have any defects. Outdoor area was in good condition. Based on the information above, adequate supervision was being provided during outdoor play time. The structure was maintained in a good condition. There was nothing staff could have done to prevent the incident from happening. No deficiencies are being cited in accordance with Title 22 Regulations. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed Lead Teacher Liza Cantabrana, at 12pm. Copy of Report provided. END OF REPORT
