What the inspector wrote

Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced Case Management inspection to follow up on an incident report submitted to the Department on 04/28/2026. Upon arrival at 2pm, LPA met with Facility Representative Carmen Nelson, to whom the purpose of the inspection was explained. A tour of the facility was provided and census was taken. There were 22 children with 6 staff members. On 04/27/2026, Child #1 (C1) was playing with peers near a concrete cinder block wall. While holding a ball, C1, stepped up on the wall. Staff #3 asked the C1 to step down, and as C1 did, they lost balance and fell. Child injured the back of their head, which resulted in further medical evaluation. Facility staff followed through with attending to injury with First Aid. Parent of child was contacted. LPA conducted interviews with two staff members who were present during the day of the inspection. Children were unable to be interviewed during today's inspection. Staff interviews corroborated what was reported on the Unusual Incident Report regarding how Child #1 got injured. Both staff stated that Staff #3 was present with the child, speaking with C1 to not climb on the wall. Per staff, children are attracted to that area and they have to remind them not to try and climb up on it. LPA observed the area where the incident took place. Pictures were obtained. Based on observation and the information gathered, facility did not ensure that area/wall that poses a hazard to children in care is made inaccessible. This is a violation of Outdoor Activity Space. The deficiencies listed on the following pages were observed by the LPA and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809D for deficiencies that are being cited and need to be cleared to protect the children’s health & safety. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Facility Representative Carmen Nelson, at 3:30pm. Plan of corrections developed and copy of Report provided. END OF REPORT
