PLAN OF CORRECTION INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced plan of correction inspection to the above facility on 08/08/2023 to ensure the Type A deficiency cited on 08/01/2023 has been cleared. LPA arrived at the facility at 1:00 PM and met with Gayane Chilingaryan, Licensee who guided analyst on a tour of the facility. A COVID 19 risk assessment was conducted prior to entering the facility. During this inspection there were 5 children present in the facility with licensee, three being infants and two children age between 2-3 years old. Per licensee, one infant was dis-enrolled since 08/01/23 and currently there are total of 5 children enrolled in the daycare, three being infants and two age between 2-3 years old. A copy of current facility sketch was provided to LPA. At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today from the areas that were inspected today. Acknowledgement forms were observed for all children in care for 07/24/23 an 08/01/23. LPA cleared deficiencies on this date and provided a copy of the Licensing Report to Licensee, and issued Plan of Correction clearance letter. The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Licensee, Gayane Chilingaryan at 2:30 p.m..
