What the inspector wrote

Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced Case Management inspection to follow up on an incident submitted to the Department on 05/07/2026. Upon arrival at 10:30am, LPA met with Facility Representative Catalina Poblano, to whom the purpose of the inspection was explained. A tour of the facility was conducted and census was taken. There were 65 children with 17 staff members. On 05/06/2026, Child #1 (C!) was playing on the middle playground, when they tried to go up the wide slide and slipped. C1 slipped and hit their mouth on the slide, cause them to need first aid and additional medical evaluation. During today's inspection, LPA conducted interviews with three staff members. Pictures of the play structure were obtained. During staff interviews, Staff #1 stated that during the incident, they were by the sandbox and had to remind C1 multiple times not to climb the slide, and then they did it again and slipped. S1 said they observed the child fall, and went over to help them up. C1 began crying and Staff #2 (S2) took child inside to provide first aid. S1 states they constantly remind the children not to climb the slide and sometimes have them show the staff how you're suppose to go up and come down. All staff state that they provide reminders throughout the day about how to use the structure. Staff #2 states they tell them "we go up the stairs, and down the slide." S2 also stated that they heard S1 remind children multiple times to not climb the slide when the incident happend. Staff state that now they are keeping a more attentive eye on the slide, staying closer, and constantly reminding the children based on what happened to their friend, so they can be safe and not hurt. Based on the interviews, and the information gathered, Staff were adequately supervising children during play and were verbally reminding children how to use the play structure. REPORT CONTINUES PAGE 1 of 2 Child #1 quickly went up the wide slide and slipped before staff could respond and redirect child down. Staff were continuing to instruct children not to go up the slide that day. The incident was too quick for staff to be able to prevent. This incident was an unusual accident and no violation is being addressed. LPA is providing an Advisory Note to the facility for supervision moving forward. At this time, no citations are being provided in accordance with California Title 22 Regulations. A notice of site visit was given and must remain posted for 30 days.
