What the inspector wrote

On October 15, 2025, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- Incident inspection at the above facility. LPA met with director Susan Wood who guided LPA on a tour of the facility. LPA observed 56 children with 14 staff members. The purpose of the inspection is to follow up on an incident that occurred on 10/06/2025 and was reported to the department on 10/07/2025. Incident report was reported timely to the department. During the inspection LPA interviewed Parent #1 (P1), LPA interviewed Staff #1 (S1), LPA interviewed Child #1 (C1) and LPA toured the area where the incident occurred. Based on interviews conducted, P1 stated that C1 told their mom during picked up time that C1 had a wood chip in their ear. Per P1 and S1, S1 was not aware there was a wood chip in C1 ear. Per S1, C1 did not display any discomfort or comment of any discomfort due to the wood chip. Per S1, C1 had a good day. Per S1, it was not until C1's mother arrival that S1 became aware of a wood chip in C1's ear. Per S1, P1 and C1, C1 was taken to the doctor where a piece of wood chip was removed from C1's ear. Per C1, they placed the wood chip in their ear. Per C1, it was a tiny piece. Per P1, there was no damage to C1 ear. LPA reminded director to ensure that all staff members are providing 100% percent supervision at all times. There are no deficiencies being cited at this time. An exit interview was conducted and a copy of this report was provided to director along with Notice of Site Visit. The Notice of Site Visit must be posted for 30 days.
