What the inspector wrote

On February 11, 2026, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- Incident inspection at the above facility. LPA met with director, Susan Woods and Site Supervisor Olivia Garcia who guided LPA on a tour of the facility. LPA observed 98 children in care with 22 staff members. During the inspection LPA interviewed Staff #1 (S1) to Staff #3 S3) and attempted to interview Child #1 (C1). LPA had previously received facility roster during the initial follow up visit of the incident on 01/05/2026. Per S3, they observed C1 walking around the tree, counter clockwise with their left hand placed on the tree. Per S3, they observed C1 step on a root of the tree and somehow slipped. Per S3, they checked on C1 and observed they were bleeding. S3 stated they called for help and another staff came to take C1 to S2. S1 stated they stayed back to ensure no children got near the area where the blood was. Per S2, they took C1 to the restroom to assist with first aid when S1 came in and assisted. Per S2, they notified the office (site supervisor) of the incident and continued to clean/disinfect the area where the incident occurred. Per S1, C1 was brought to them by S2 and stated that C1 was bleeding from their mouth. S1 stated that they applied pressure to the area and once the bleeding stopped applied an ice pack. Per S1, C1 was brought into the director's office with the assistance of sit supervisor. S1 stated that while entering the office building C1's mother had arrived at the facility as it was during pick up time. S1 and S2 stated that the facility contacted their 5000 (the university's 911) who also came into check on C1. At this time there are no deficiencies being cited at this time as the incident appears to be an accident that occurred quickly and was unable to be prevented. An exit interview was conducted and a copy of this report was provided along with Notice of Site Visit that must be posted for 30 days.
