What the inspector wrote

On December 5, 2024, Licensing Program Analyst (LPA) Staicy Perry conducted an unannounced case management inspection to follow up with an incident that was reported to the department. LPA met with Yvonne Hernandez, Director, who guided LPA on a tour of the facility. Census was taken. This program is licensed to care for children aged two until the entry to first grade. Also on premises, infant licensee #198017960. On 11/26/2024, it was reported to the department that on 11/25/2024, C#1 was running outside and hit C#1 forehead on the platform of the play structure causing C#1 to get half an inch cut across C#1 forehead. First aid was provided to C#1. Facility called 911 as a precautionary measure and child was transported to the hospital to seek emergency medical treatment. C#1 parents were notified of the incident in a timely manner and C#1\342\200\231s parents arrived to join C#1 on the ambulance to the hospital. Per Yvonne, C#1 received stitches and returned to school on 12/5/2024 with no restrictions. The incident was reported to the Department within the 24-hour period. Interviews conducted with children and staff stated that the incident was observed, the incident happened so fast it was unable to be prevented. LPA Perry did obtain and review documents pertaining to the incident. Based on information obtained and interviews conducted there was no corroborating information to determine that there was a lack of supervision during the incident that occurred on 11/25/2024 was observed and first aid was provided, and it was determined to be an accident. Currently the facility is in accordance with California Code of Regulations Title 22. No deficiencies will be cited on this date. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Yvonne. Appeal rights discussed and provided. END OF REPORT
