Based on observation and record review, the licensee did not comply with the section cited above in that licensee was over the capacity caring for 3 infants and 4 preschoolers alone without an assistant which poses an immediate health, safety or personal rights risk to persons in care.

Documentation shall be maintained in the infant’s file and be available to the Department ...shall include the following: Date, Infant's name, Time of each 15-minute check. This requirement is not met as evidenced by: Per file review, LPA found that Child #1 and #2 were missing the LIC 9227 and Infant sleep log. Per licensee, they were unaware of new Safe Sleep requirement.This poses a potential risk to the health and safety of children in care.
