What the inspector wrote

Licensing Program Analysts (LPA) Anomeh Eivazian and Mary Silva conducted an unannounced case management inspection on 03/27/23 at 12:30 PM, due to an incident that occurred on Monday 03/27/23. LPAs met with Celine Abranians, Assistant Director who guided analysts on a tour of the facility. Alleged Incident took place on 02/13/23. Incident was reported via email on 03/06/23. LPAs observed the area where alleged incident took place. LPAs conducted interviews with staff. LPAs obtained a copy of child#1's February 2023 attendance sheer and email communication between school and parent#1. Based on an interview that was conducted with staff#1, on 02/13/23 when parent#1 picked up child#1, there were no other children outside. LPAs issued the Confidential Names List (LIC 811) to the assistant director during this inspection. The Confidential Names List documents the staff and children involved with the incidents documented in this report. At this time, there is not a preponderance of evidence that shows that the facility was in violation with Title 22 Regulations when these incident occurred. Therefore, there are no deficiencies being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Assistant director, Celine Abranians, at 2:30 PM.
