What the inspector wrote

At 12:31PM, Licensing Program Analyst (LPA) Steven Tung conducted an unannounced case management visit regarding an unusual incident report submitted to the Department on 10/12/2022. The unusual incident report from the facility stated that a child was left unsupervised outdoors on the playground for 5 minutes. LPA met with Director Betty Castillo who led LPA on a tour of the facility. Census was taken. LPA obtained class roster, class schedule, and sign-in sheet as well as conducted interviews with staff and child. Interviews and statements obtained from Staff and Child confirmed the incident. Staff providing care and supervision disclosed being in a separate area with the classroom children while the child involved was left alone without supervision on the playground - this poses a potential health and safety risk to children in care. The following deficiency listed on the attached LIC 809 deficiencies page is being cited in accordance with California Code of Regulations Title 22. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Betty Castillo.
