Licensing Program Analyst (LPA) T. Tran arrived at Apollo Center to conduct a Case Management inspection that was self-reported on 06/07/2023 regards to the level of care and supervision. Upon arrival, LPA met with teacher Aracely Marquez and we toured the facility. LPA observed proper care and supervision. LPA completed file review for staff and children. LPA obtained personnel report, child's document, and doctor's note. Interview was conducted with staff and children. Based on the information that was gathered, there were 8 children present with two staff when the incident occurred. Parent was contacted immediately. Based on the available information it does not appear this incident was the result of a Title 22 violation for care and supervision. No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Aracely Marquez.
