What the inspector wrote

On 04/04/2024 at 10:15 AM Licensing Program Analysts (LPAs) Katrina Chicote and Claudia Kam conducted an Unannounced Case Management inspection to follow up on an incident that was reported to the Department on 03/11/2024 . Upon arrival, LPAs announced purpose of visit and met with Candeleria Muniz, Facility Representative (FR). Census was taken. On 03/11/2024, an incident was reported to The Department where a child fell outside and hit their head causing an open wound that resulted in child being taken to the doctor . Child returned following school day with no restrictions. Facility reported the incident the following Monday and submitted written report within seven days per Title 22 Regulation reporting requirements. During this inspection, LPA interviewed staff and confirmed staff observed incident and provided first aid immediately and Authorized Representative was notified immediately via phone call. LPAs reviewed written notification that was also provided to parent which includes Head Injury report that is confirmed signed by Authorized Representative. LPAs observed play area outside during inspection and did not observe any immediate hazards of area. Based on the information obtained and LPAs observation of the play area, LPAs determined there was adequate supervision and immediate response was provided to child. The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 04/04/2024. A notice of site visit was given and must remain posted for 30 days.
