What the inspector wrote

Licensing Program Analyst (LPA) T. Tran conduct an unannounced visit at Appollo Center to follow up on a Case Management Incident that was self-reported on 02/19/25. The Monterey Park Southwest Regional Office received the incident report on 02/21/25. Upon arrival, LPA met with lead teacher, Candelaria Muniz. LPA toured the facility and observed proper care and supervision. Today\342\200\231s census is 21 children with 7 teachers. All center staff that were present during today\342\200\231s inspection had fingerprint cleared and associated to the designated license number. LPA obtained child's document and personnel report. LPA conduct interviews with staff and attempted interview child and others. Based on interviews conducted, it was revealed that on the day of the accident there were two teachers supervising a child. While the child was playing with the manipulative toy at the table, as the child got up then the foot was caught at the table leg. Child sustained a cut on the lower lip. The child\342\200\231s parent was present when the incident occurred. Staff immediately cleaned the cut and applied ice pack. At this time, based on the available information it does not appear this incident was the result of a Title 22 violation for lack of care and supervision. No further action is required. The facility was found in compliance with Title 22 regulations, there will be no deficiencies cited during today's visit. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with the facility representative, Candeleria Muniz.
