What the inspector wrote

On December 8, 2025, Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced Case Management Inspection – Plan of Correction for the above facility. LPA was greeted by the facility representative, Rocio Velazquez, who guided LPA on a tour of the facility. This is a preschool program that operates Monday through Friday from 7:45 am to 4:15 pm. Per facility representative, there are 40 children enrolled, and LPA observed 29 children in care along with 8 staff members. The purpose of this inspection is to ensure that the facility is in compliance with Title 22 Regulations and the deficiencies cited on 11/07/2025 were corrected. Licensing staff observed and reviewed the following: Staff 1 obtained proof of health screen, immunization record and Tuberculosis Test (TB) Staff 2 obtained proof of immunization record Letters of Deficiencies Citations Cleared were provided for deficiencies corrected. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Rocio Velazquez. Page 1 of 1
