What the inspector wrote

On February 26, 2026, Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced case management inspection for the above facility. LPA was greeted by the Site Supervisor, Jazmin Leal, who guided LPA on a tour of the facility. The purpose of this inspection is to follow up on a self-reported incident that occurred at the facility on 01/16/2026. This is a preschool program that operates Monday through Friday from 7:45 am to 4:15 pm. Per Site Supervisor, there are 43 children enrolled, and LPA observed 34 children in care along with 6 staff members. Brief summary of incident: Child #1 (C1) was playing on the cement area on the play yard when they slipped, fell and hit the back of their head During this visit, LPA interviewed C1(C1). During the course of the this investigation LPA interviewed Staff 1 (S1) - Staff 3 (S3), observed the cement area where the incident occurred and reviewed a picture that was taken of the cement area right after the incident occurred. Based on interviews, C1 expressed that they were playing with a basketball when they slipped, fell and hit the back of their head. C1 mentioned, S2 and S3 were outside when they fell and S2 help them up. S2 mentioned that they witness C1 slip and fall on the cement area while they were trying to retrieve a basketball. C1 expressed that their shoes were slippy which causing them to fall. Per C1 and S1 - S3, C1 received an ice pack after the incident. Per S1 - S3 the parent was called and C1 was pick up with in 20 minutes of incident. C1 was observed by a medical professional and returned to the facility on January 19th with no restrictions. Page 1 of 2 LPA inspected the play yard and did not observed any hazardous materials or water that caused C1 to slip and fall. LPA also reviewed a picture that was taken right after the incident of the cement area and LPA did not identify any hazardous materials or water that caused C1 to slip and fall. Based on interviews and observations conducted there was no corroborating information to determine that there was a lack of supervision during this incident. This incident was deemed an accident. All reporting requirements were met. There are no deficiencies being cited at this time. A notice of site visit was given and must remain posted for 30 days Exit interview conducted and report was reviewed with the Site Supervisor, Jazmin Leal. Page 2 of 2
