What the inspector wrote

Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced Case Management inspection due to an incident that was reported to the Department. LPA met with Site Supervisor Cathy Gaeta who guided LPA on a tour of the facility. Census was taken. On September 24th, 2025, an incident was self reported to the Department via Email by the facility who reported that on September 23, 2025 a child was left unsupervised in a classroom. Report was reported within the required 24 hours. The purpose of the inspection was to obtain additional information regarding the incident reported to the Department. During the inspection, LPA Lopez conducted interviews with Site Director and staff. During interviews it was disclosed that during breakfast around 8:30 am child was falling asleep at the table, staff decided to move child onto the quiet area where child feel asleep, authorize representatives were called. Breakfast transition continued, teacher and teacher assistant took children to the restroom located on a hallway located outside of the classroom, leaving sleeping child behind. Child was found in the classroom by an intern. Per Site Supervisor child was left unsupervised between 5-10 minutes. Child was picked up by authorize representative at 9:02 am. LPA Lopez was unable to interview the teacher and teacher assistant who left the child unsupervised as they were not available for interview. Interviews conducted disclosed that resulted in lack of supervision which is an immediate danger for the health and safety of children in care.----------------------------- pg 1 of 1 ----------------------------------- Based on the LPA\342\200\231s observations and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code od Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health and safety. LPA Roxana Lopez informed facility representative Cathy Gaeta that this report dated 10/2/2025 document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Roxana Lopez informed the facility representative to provide a copy of this licensing report dated 10/2/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days.
