On December 3, 2025 at 11:20 am Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced annual inspection at the above facility. LPA was greeted by Licensee, Marine Tovmasian who guided LPA on a tour of the facility. Licensee\342\200\231s husband and co-licensee were also present during the inspection. Per licensee there are 13 children enrolled, and LPA observed 11 children in care. Hours of operation are Monday \342\200\223 Friday from 8:00 am \342\200\223 5:00 pm. Family members residing in the home are 4 adults who obtain criminal record clearance. This is a single-story house with 3 bedrooms, 2-bathroom, kitchen, dining room, living room, family room, front yard and completely fenced rear yard. The facility also has a back house with a different address (4414 Camero Ave Los Angeles, CA 90027), which is off limits to children in care Off limit areas are 2 bedrooms, 1 bathroom, kitchen, dining room, living room, front yard and back house. Areas accessible to children in care are the family room,1 bedroom, 1 bathroom located in bedroom 1, and back yard (fenced). At 11:20 am the licensee guided LPA on a tour of the facility. Per licensee, parents enter through the drive way which leads them to the family room, which is the main daycare area. Upon entrance to the family room, LPA observed multiple age appropriate toys and supplies for children in care. LPA was then guided to the bedroom that is directly next to the family room which is used as an napping area. Napping equipment in the form of 4 cribs and multiple cots were observed. LPA observed the cribs to have a blanket tied to the side of cribs. Per licensee, these blankets were placed in this manner, so the children did not get distracted during nap time. Page 1 of 4 LPA had licensee remove the blankets as they obstructed view of children when placed on the crib and reminded licensee that nothing should be attached to the side of the cribs. Each crib had an appropriate size mattress with fitted sheets. LPA also observed an infant in a crib using their pacifier and attached to the pacifier was a mini stuffed animal. LPA brought this matter to the licensee\342\200\231s attention and they removed the stuffed animal from the pacifier right away. Bedding was inspected for good condition, appropriate storage, and cleanliness. Per licensee, all bedding is washed at the facility weekly. The bathroom that is located inside bedroom 1 was then inspected. The bathroom was clean, orderly and LPA did not observe any hazardous materials that could pose a danger to children in care. Licensee then walked LPA to the backyard, which LPA observed a bolted play structure suitable for children in care as well as multiple age appropriate toys. All areas were observed to be clean, orderly, comfortable, and well ventilated. There are first aid supplies available. Per licensee, the facility provides all meals for children in care. LPA reminded licensee that food brought from the children's homes, the container shall be labeled with the child's name and properly stored or refrigerated. If a child becomes ill, the child is isolated in an area in the family room away from the other children while the parents are called. A crib/cot is available if needed. Licensee's poisons, detergent, cleaning compounds, medications and other items which could pose a danger to child are stored where they are inaccessible to children. LPA observed a smoke/carbon monoxide detector in the family room, however, did not test as children were napping. LPA observed a 2A-10BC fire extinguisher in the family room. The valve on the fire extinguisher pointed to the fully charged green section with a service date of 9/09/2024. Per licensee, the company who services the fire extinguisher has been called and will be coming to service the fire extinguisher this month. There is a working telephone on site. Per Title 22 Emergency Disaster Drill must be conducted and documented at least every 6 months. LPA observed the last emergency disaster drill documented on 10/16/2025 Per the licensee, there are no weapons, or bodies of water on the premises. LPA did observe one small dog to be house in the back house, which is not assessable to daycare children. Licensee obtains the current Pediatric First Aid and CPR certification which expires on 05/01/2026. LPA conducted a children file review and deemed it complete. LPA conducted staff file review and deemed it complete. Page 2 of 4 Licensee has completed mandated reporter training which expires 06/30/2027. All childcare employees must complete mandated reporter training within 30 days of hire and renew training every two years at www.mandatedreporterca.com Licensee has the required documents posted in the FCCH; Facility License (LIC 203), Notification of Parents' Rights Poster (PUB 394) and Earthquake preparedness Checklist. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the license, Marine Tovmasian confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health and safety. A notice of site visit was given and must remain posted for 30 days.
