What the inspector wrote

Licensing Program Analyst (LPA) Lilli Babcock conducted an unannounced Case Management inspection due to an incident that occurred on 3/26/26 and was reported to the Department on 3/27/26. LPA met with Licensee, Susana Alvarado to whom the reason for the visit was explained. Licensee guided LPA on a tour of the facility. Census was taken. Present during the inspection were Licensee, Assistant, and two children (Licensee's grandchildren). Licensee states there are 15 children enrolled. The facility was observed to be operating within the license capacity limitations. On March 27, 2026, an unusual incident report was made to the Department regarding an incident that involved a child (Child #1) (C1) who sustained an injury that required medical attention. The facility reported this incident to the Department within the required timeframe. Based on information obtained during interviews conducted with Licensee and staff, LPA Babcock determined that during outdoor play C1 was riding a tricycle on the patio in the backyard. Staff #1 stopped C1 from riding the tricycle as their helmet appeared loose. Staff #1 was holding the tricycle when C1 was sitting on the tricycle trying to adjust their helmet. C1 leaned back and fell to the ground hitting the back of their head on the concrete and then had a seizure. Licensee and Staff #1 did not observe any apparent bump, scratch, or injury to the back of the head of C1 after the fall. Per Licensee and Assistant there were 10 children and three staff present at the time of the incident. Page 1 of 2 C1 was taken to the hospital after the seizure. C1 was not at the facility on the date of LPA visit and therefore could not be interviewed. At 1:00 pm. during this inspection, LPA inspected the outdoor area and did not observe any hazards near or around the area where incident took place. LPA inspected the tricycle and helmet C1 was wearing at the time of the incident. Licensee provided LPA with a printout with the details and dimensions of the tricycle and item details for the helmet. LPA observed the printout shows the tricycle is recommended for ages 2-4, which is within the age range of C1 and the helmet is for age range of "Toddler/Little Kid". No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Licensee, Susana Alvarado. Page 2 of 2
