Based on observation, record review, and interview, the licensee did not comply with the section cited above as all staff do not have a current Mandated Reporter Child Care Provider (AB1207) Training, which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above in 1 of 2 staff files, Staff #2, did not have a current Mandated Reporter Child Care Provider (AB1207) proof of completion which poses a potential health, safety or personal rights risk to persons in care.

Family Day Care Homes

1597.622(a)(1)a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles.This requirement was not met as evidenced by: Based on interview, Licensee did not ensure that there were records of the Staff #2 immunization against influenza, pertussis, and measles prior to working and residing in the home. This poses a potential risk to the health, safety, and personal rights of children in care.

"102416(d) Personnel RequirementsPrior to employment or initial presence in the child care home, all employee...shall: (1) Obtain a California clearance or a criminal record exemption as required by law or Department regulations.... This regulation was not met as evidenced by:" Based on observation, interview, and file review, Licensee did not ensure that Assistant Li Zhang had criminal background clearance prior to working and residing at the facility. This poses an immediate risk to the health, safety, and personal rights of children in care.

102416(c)(1)(b) Personnel RequirementsA licensee of a large family day care home shall ensure that at least one person who has a current certificate in pediatric first aid and pediatric cardiopulmonary resuscitation shall be available at all times when children are present at the facility, ....This requirement was not met as evidenced by: Based on interview and file review, Licensee did not ensure that her Pediatric CPR and First Aid Training was current. The Certificate notes expiration 12/2020. This poses a potential risk to the health and safety of children in care.

APPLICATION FOR INITIAL LICENSE

102369(b)(9) Application of Initial LicenseEvidence of a current tuberculosis clearance, not more than one year prior to or seven days after initial presence in the home, for any adult in the home during the time that children are under care. Based on interview and file review, Licensee did not ensure that Staff #2 had record of negative TB Testing in their file. This poses a potential risk to the health and safety of children in care.
