Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced Case Management Incident Inpection to follow up on an incident that was reported to the Department on 7/31/25, LPA met with Licensee Ludivina Renteria who the purpose of the inspection was informed. Census was taken. On 7/31/2025, an incident was self-reported to the Department via email by the facility who reported that child # 1 had fallen hitting their lip and chin on a blue small couch and child was taken to the doctor for a check up. Report was reported within the required 24 hours. The purpose of the inspection was to obtain additional information regarding the incidents reported to the Department. Incident was observed by Licensee, child was walking towards the small couch, tripped hitting their lips and chin on the wooden part of the couch. The bump cause a cut on the lip and brusing. Area was cleaned and ice was applied. Licensee called parents and child was taken to the doctor for a check up. Cut did not required stitches child returned to the facility the following day. At this time, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days.
