What the inspector wrote

Licensing Program Analyst (LPAs) Thelma Razo and Rita Ramos conducted an unannounced Required – 1 Year inspection on 7/8/2021 at 9:30AM. LPAs met with Program Manager Sheena Minaya who guided analysts on a tour of the facility. This is a preschool program which consists of 3 classrooms; Room 101, Room 103 and Room 105. Facility operation hours are Monday to Thursday from 7:15AM to 5:30PM. This facility also has an infant component on the premises license #198018561. All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection: Room 101: Staff #1, Staff #2, Staff #3 with 8 preschoolers; Room 103: Staff #4, Staff #5, Staff #6 with 8 preschoolers; and Room 105: Staff #7 and Staff #8 with 8 preschoolers. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility: Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Children bring their own blankets and parents take them home on Fridays to be washed. There are no laundry services available on site. Napping equipment was observed in each classroom (mats). Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms in Room 101, 103 and 105, however, LPAs observed live maggots by the sink in Room 105 and one of the 4 faucets in Room 105 is not operable. Availability of indoor drinking water was observed in classrooms. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Program Manager states there are no poisons at the facility. Carbon monoxide detectors were observed and are operable. PAGE 1 OF 4 All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is shade in the play yard. Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time. All floors were observed to me clean and safe. Al materials accessible to children were observed to be toxic free. There are no firearms stored on the premises. There are no pools or bodies of water at the facility. All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Children’s Records were reviewed. (Name, address, telephone of child’s authorized representative, Medical Assessment.) for completeness; Inspection of required forms was made and documented on the LIC 857. LPA also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present had proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present had proof against TB, measles, pertussis, and influenza. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and PAGE 2 OF 4 appropriateness to children in care. The facility provides breakfast, lunch and a PM snack. Meals are individually served to children in care via the use of disposable containers and utensils. First Aid supplies were observed in each classroom. The facility was observed to be equipped with an isolation area for any child who becomes ill and it is located in the main office. Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. UPDATE : LPA advised Program Manager of the Effects of Lead Exposure form during the inspection and discussed the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD. There are citations being issued at the time of inspection under California Code of Regulations, Title 22 Division 12. Refer to LIC809-D. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Program Manager was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit. A copy of the Parent Notification Requirements was also provided to the Program Director. Page 3 of 4 The Notice of Site Visit (LIC 9213) was provided – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Facility Representative. Appeal Rights explained & provided together with the Facility Evaluation Report. PAGE 4 OF 4

    Fixtures, Furniture, Equipment and Supplies.All toilets, handwashing and bathing facilities shall be maintained in safe and sanitary operating condition. Additional equipment, aids and/or conveniences shall be provided as needed in centers that serve children with physical disabilities. This requirement is not met as evidenced by: LPAs observed that the faucet in Room 105 is not operating. This poses a potential health and safety risk to children in care.
