What the inspector wrote

On 12/02/2025 Licensing Program Analyst (LPA) Mariah Aguirre conducted a Case Management inspection Deficiencies. During today's visit, LPA conducted interviews with Staff #1 (S1)- Staff #3 (S3). LPA was unable to interview Children due to the age and lack of verbal ability of children involved. During interviews, S1 disclosed that they do swaddle infants in care when they feel it will assist with their sleep. Both S2 and S3 stated infants are swaddled sometimes. The following deficiencies listed on the attached 809D deficiencies page are being cited in accordance with California Code of Regulations Title 22. LPA Mariah Aguirre informed licensee Rosie Sanchez that this report dated 12/02/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health and safety of children in care. LPA Mariah Aguirre informed licensee Rosie Sanchez to provide a copy of this licensing report dated 12/02/2025 that documents one type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), must be placed in the child's file for verification. A copy of this report along with appeal rights were given to licensee Rosie Sanchez and a Notice of Site Visit was provided and must remain posted for 30 days. Page 1 of 1
