What the inspector wrote

On June 2, 2026, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- Incident inspection. LPA Director, Aracely Ramirez who guided LPA and Investigator on a tour of the facility. LPA observed 14 children with 3 staff. The purpose of the inspection is to follow up on an incident report that occurred on 05/01/2026 and was reported to the department on 05/04/2026; the incident was reported timely. LPA conducted initial inspection on 05/15/2026. During the inspection, LPA interviewed Staff #4 (S4) and obtained contact information for Staff #5 (S5). LPA was unable to interview children as children were napping. LPA obtained a current facility roster. There are no deficiencies being cited at this time. An exit interview was conducted and a copy of this report was provided to director, Aracely Ramirez.
