What the inspector wrote

On October 22, 2025, Licensing Program Analyst (LPA) Staicy Perry conducted an unannounced Case Management- Incident visit to the above facility. The purpose of the visit was in response to an incident reported to the Department regarding a lack of running water at the facility on September 19, 2025. Upon arrival, LPA Perry acknowledged that the licensed childcare facility is located on an elementary school campus, Northrup Elementary School. Access was granted, and the facility was located to the left of the main office building on the left side of the campus. LPA Perry met with Education Coordinator, Dominique Pastrana who took LPA Perry on a tour of the facility and a census was taken. The facility was observed to be operating within the capacity stated on their license. It was reported that on Friday, September 19, 2025, at approximately 10:10 AM, teachers became aware that the facility had no running water. Teachers notified the Educational Coordinator, who immediately contacted parents for pick-up. All parents were promptly notified, and al l 23 chi ldren present were picked up. There were five (5) staff members on site at the time. Maintenance personnel was contacted and able to repair the issue. The facility resumed normal operating hours on Monday, September 22, 2025 (8:15 AM \342\200\223 2:45 PM). During the outage, staff and children utilized hand sanitizer and a filtered water dispenser available inside the classroom to maintain health and safety standards. The facility submitted an Unusual Incident Report (LIC 624) in a timely manner. The Department received the LIC 624 on September 19, 2025, at 1:18 PM via the MPRO inbox per title 22 regulations. LPA clarified that the facility\342\200\231s profile phone number listed with the Department was not functional. LPA attempted to call multiple times at the time of the incident reported and was unsuccessful in reaching facility. During this inspection, the Educational Coordinator provided updated contact information (phone: 562-551-6847 and email: dpastrana@pdlr.org). LPA Perry reminded the facility that any changes to contact information must be reported to the Department per Title 22 regulations reporting requirements. All staff present were associated to the facility and had fingerprint clearances in an administrative file under facility number #191570774 (Lakeview). Facility is currently in compliance with Title 22, regulations. No deficiencies were observed during today\342\200\231s inspection. Exit interview was conducted with the Educational Coordinator, Dominique Pastrana. A copy of this report was provided. Notice of Site Visit was provided and informed that the report must be posted for 30 consecutive days.
