What the inspector wrote

Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced case management inspection due to deficiency observed. LPA met with Sithara Warnakula Madushani, Staff/Facility Representative and toured the facility. There were 8 children present including 2 infants. During this visit LPA observed Child #1 (age 2yrs. 11mos)) napping in infant rocker (photo taken), Child #2 (age 2yrs. 4mos) napping on a couch supervised closely by Staff #2. Posing a potential risk to the health and safety of children in care. The facility representative, removed child #1 and 2 and placed them on mats for napping. Exit interview conducted with Licensee, Hewa De Silva. Appeal Rights provided. The Notice of Site Visit and report was issued. Notice of Site Visit must remain posted for 30 days.
