What the inspector wrote

Licensing Program Analysts (LPA) Alicia Mooberry conducted a Case Management Visit for the purpose of Deficiencies cited on 10/29/25. LPA met with Facility Representative Bubudu Karralliyadde. LPA toured the facility and observed 9 children including 1 infant supervised by Hermelinda Nunez and Dilani Alawi. The purpose of the inspection was to ensure the Licensee has corrected the deficiency cited on 10/29/25 . During this inspection LPA reviewed staff file and observed the following: Staff Dilani Alawi's file contained LIC 9052, LIC 9108, and evidence of TB test LPA observed infant napping in crib that has no lose items, following safe sleep regulations Based on LPA records review and observations, deficiencies previously cited have been cleared. LPA issued Letter of Deficiencies Cited Cleared during visit. No Deficiencies were cited during this visit. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Facility Representative Bubudu Karralliyadde.
