ANNUAL / RANDOM INSPECTION CONDUCTED
On May 5, 2026, at 1:45 PM, Licensing Program Analyst (LPA) A. Carter conducted an Unannounced Annual / Random Inspection and met with the licensee Luz Felix Medina

LPA disclosed the purpose of the inspection and was granted entry into the facility. LPA was taken on a guided tour of the facility and provided licensee with a copy of the Facility Entrance Checklist.
Adults in the home were discussed, and all have criminal record clearance. Also present was Licensee’s Assistant. At the start of the visit there were 4 children present and currently 15 children enrolled. The capacity specified on the license is the maximum number of children for whom care may be provided at any one time. The children's roster was reviewed by LPA and is current. Facility provides care to children ages 0 to 13 years old and is open for 24 hours, Monday - Friday. The Licensee was advised care cannot be provided for a continuous 24 hour period for any one child.
At 3:37 PM, Assistant arrived with 1 school-age child
This is a single story home which consists of 3 bedrooms, 2 bathrooms, and detached garage.
Care is mainly provided in the living room.
Areas accessible to children are
: hallway bathroom, and (1) bedroom used as activity room.
Areas off limits to children and parents includes
kitchen / dining area, (2) bedrooms, laundry room, a restroom located past the kitchen, and garage. The licensee does understand that licensing staff may have access to off-limit areas during inspection visit if necessary. LPA observed a safety gate at the kitchen entrance separating and safety knobs on doors making them inaccessible to children in care.

All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. LPA observed a screened wall heater in the hallway. LPA did not observe any fireplaces in the home. LPA observed ceiling fans and A/C wall units for ventilation. The facility maintains telephone service via landline and cell phone.
Facility License, Emergency Disaster Plan, and Parents’ Rights were posted and observed at time of inspection. Disaster drill log was available during today’s inspection with last disaster drill conducted on 04/16/26. LPA observed the required 2A10BC fire extinguisher with service tag dated: 02/27/25. The Licensee was reminded; fire extinguishers should be purchased new or serviced annually. Licensee stated that they believed the fire extinguisher was to be serviced in June, technical violation cited. Smoke and carbon monoxide detectors were observe and tested at time of inspection. First Aid kit was observed by LPA in main care area.
LPA observed main care area has age-appropriate toys, free of loose and sharp parts. LPA observed 2 play yards against the wall not blocking entrances or exits and child size tables and chairs. Children nap in the day care room and they are provided with appropriate napping equipment.
Children use the bathroom located in the hallway. LPA observed bathroom to have operable toilet/sink and adequate toileting supplies for children in care. Personal items were observed in the cabinet under the sink secured by a safety lock. No hazards observed.
LPA observed a safety gate at the kitchen entrance making it inaccessible to children in care. Food is provided for children in care. LPA reminded licensee that any food brought from the children’s homes shall be labeled with child’s name and properly stored or refrigerated.
Detergents, cleaning compounds, medications, and other items which can pose a danger to children are inaccessible stored in off-limit areas of the home. The Licensee states that there are no poisons in the home and understands that poisons must be locked.

Per licensee, the children have access to the backyard for outdoor play. LPA observed a variety of age appropriate toys available to children in care. Per Licensee, children are supervised while playing outside. LPA observed yard has grass and adequate perimeter fencing through-out the property. Play area is free of loose and sharp articles or objects and all trees, shrubs, and plants are maintained. There is 1 pet dog located on the side of the garage, inaccessible to children in care.
Infant Care:
Currently (1) infant over 12 months enrolled. LPA reviewed sleep log. The Licensee was advised; infants shall not be swaddled in care. Car seats shall only be used for transportation purposes and shall not be used for sleeping. LPA discussed the Safe sleep regulation, including LIC 9227 Infant Sleep Plan for infants under 12 months, 15-minute sleep check documentation for infants 0 -24 months.
—Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, and documentation of 15-minute Infant Sleep Check (0-24 months). All required licensing forms are in file.
Staff records were reviewed:
Licensee Pediatric CPR / First Aid valid until: 01/10/27 and assistants’ Pediatric CPR / First Aid valid until: 01/24/27. Staff had proof of required immunizations including proof of TB clearance. All adults had LIC 508 criminal record statement and LIC 9052 on file. Mandated Reporter training for the License and assistant was completed and valid until: 05/05/27.
The following was discussed:

Rooms that are off-limits need to be made inaccessible during operating hours. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers, or any other item that falls into that category are permitted in the facility.

Effective January 1, 2010, Licensees of family childcare homes are required to ensure that at least one staff member with current training in Pediatric First Aid & CPR is on site at all times when children are present.

Reporting Requirements: Changes should be reported to the Department as soon as they occur, such as construction, remodeling, telephone number changes and/or if you move from your home. Any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing.

The licensee confirms:
The isolation area for sick children waiting to be picked up is in the main care area
The facility does not currently administer medication.
There is one pet dog in the home that does not interact with day care children
There are no smokers in the home
There are no weapons or firearms in the home
There are no pools, spas or other bodies of water on the premises.
During the inspection, children present were observed to be treated with dignity and respect, observed to be receiving safe, healthful, and comfortable accommodations, furnishings, and equipment, and free from corporal and/or unusual punishment.
Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.
During the exit interview, the Licensee Luz Felix Medina, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
