Licensing Program Analyst (LPA) T. Tran and IB, M. Gliaudys conducted an unannounced Case Management Incident visit at MAOF- Hawaiian Garden Center for the purpose of following up on an unusual incident occurred on 10/09/25. Upon arrival, LPA and IB met the site supervisor, Fabiola Da Rosa and we toured the facility. LPA completed staff and children files review. Interviews were conducted with staff, children, and other. Based on the record reviewed and interviews conducted. On the day of the incident, there were 15 children with 3 staff. According to the available information, there were no witnesses that the alleged staff mishandled any children. No further action is required for this incident. No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Fabiola Da Rosa.
