Correction check·June 24, 2025

On June 24, 2025, at 2:20 pm Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced Case Management Inspection \342\200\223 Plan of Correction at the above facility. LPA met with Director, Veronica Brenes, who guided LPA on a tour of the facility. LPA observed 16 children in care along with 5 staff members. The purpose of this inspection is to ensure that the facility is in compliance with Title 22 Regulations and the deficiencies cited on 6/11/2025 were corrected. Director contacted LPA to advice that the necessary corrections have been made before Plan of Correction date of June 27, 2025 Licensing staff observed and reviewed the following: \302\267 Staff 4 (S4) has obtained proof of Tuberculosis test (TB) for LPA to observe Letters of Deficiencies Citations Cleared was provided for deficiencies corrected and no additional deficiencies were cited during inspection. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00 An exit interview was conducted, and a copy of this report was provided to Director, Veronica Brenes. Page 1 of 1
