What the inspector wrote

Licensing Program Analyst (LPA) Stephanie Li conducted an unannounced Case Management inspection due to an incident that occurred on 3/23/2026 and was reported to the Department on 3/24/2026. LPA met with Education Coordinator Denise Manaloto. Reason for the visit was explained. There were 30 children and 5 staff. On 3/23/2026, an unusual incident report was made to the Department regarding an incident where a child was injured while pulling running outdoors, child tripped and fell landing on his arms. Later parent disclosed that child had a cast and has restrictions to implement. During todays visit, LPA interviewed teacher regarding the incident, observed outdoor play area where child fell. No hazards were observed. Updated doctors note was reviewed. Cast has been removed from child’s arm but he still has restrictions and a sling on his arm. LPA observed group of children are older and a very active group. There is efficient open space for running, riding and games. LPA advised to organize active areas of play with visual aids and boundaries. No follow up is necessary at this time. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Education Coordinator Denise Manaloto.
