On June 19, 2025, Licensing Program Analyst (LPA) Staicy Perry conducted an unannounced case management inspection to follow up on a Plan of Correction related to a Type A deficiency issued on June 9, 2025, under Title 22, Regulation 101229(a)(1) Responsibility for Providing Care and Supervision. LPA Perry met with the facility Director Cynthia Meza upon arrival. A census was conducted and LPA Perry observed 60 children in care at the time of the visit with 15 staff present. LPA reviewed and obtained documentation of a staff meeting held on May 23, 2025 including a sign-in sheet with staff signatures, verifying attendance and discussion of supervision practices. LPA observed the emergency exit door in the preschool yard, which had previously been involved in the cited incident. A photo was taken showing a metal cover installed over the latch to render it inaccessible to children. This corrective action was completed on May 22, 2025. Per the Director, staff continue to implement a plan ensuring active communication and coordination in outdoor areas to maintain 100% supervision, particularly in accordance with children’s chosen play areas. Based on documentation review, staff meeting, and visual observation, the deficiency cited on June 9, 2025, has been cleared. LPA Perry provided the facility with a Letter of Deficiency Citations Cleared. This letter must be retained on file for three (3) years and made accessible to the public upon request, per Division 12, Chapter 1 Regulations. No deficiencies were observed during today’s visit. LIC 9213 – Notice of Site Visit was issued and must be posted for 30 days. Appeal rights have been discussed and provided.
