On March 3, 2026, Licensing Program Analyst (LPA) Staicy Perry conducted an unannounced case management inspection to follow up on a Plan of Correction related to a Type B deficiency issued on 2/23/2026, under Title 22, Regulations 101212 Reporting Requirements and 101212 Reporting Requirements . LPA Perry met with the facility Director Cynthia Meza upon arrival. A census was taken and LPA Perry took a census was taken and LPA observed 58 children in care. . LPA reviewed and obtained documentation for (1) Type B violation, Title 22 Regulations. Director Cynthia provided LPA Perry with a printed copy of the regulations with a date and signature acknowledging that director Cynthia reviewed and read and understands her reporting requirements as well as a declaration of understanding the steps of the LIC 624 form once an incident has been submitted. Director did call and submit LIC 624 form to report the incident. In addition, LPA reviewed and obtained documentation for (1) Type A violation, Title 22 Regulations. LPA obtained an email that was sent out to all facility staff on Friday 2/20/2026 reminding all staff to maintain accurate attendance records. LPA Perry did also observe sign in and out sheets have been moved from the front office to each classroom to ensure ratios and supervision is being met and that staff are aware of who is in their care at all times. Director Cynthia also had a review of Title 22 Regulations 101229 Responsibility for Providing Care and Supervision and had staff sign, date and acknowledged they understood the regulation. Based on documentation review, LPAs observations and visual documentation received, the deficiency cited on 2/23/2026, has been cleared. LPA Perry provided the facility with a Letter of Deficiency Citations Cleared. This letter must be retained on file for three (3) years and made accessible to the public upon request, per Division 12, Chapter 1 Regulations. No deficiencies were observed during today’s visit. LIC 9213 (Notice of Site Visit) was issued and must be posted for 30 days. Appeal rights have been provided. Exit interview conducted with Cynthia Meza, Director.
