On March 27, 2026, Licensing Program Analyst (LPA) Staicy Perry conducted an unannounced case management inspection to follow up on a Plan of Correction related to a Type A deficiency issued on 3/6/2026, under Title 22, Regulations, 101229(a)(1) Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections. LPA Perry met with the facility Director Cynthia Meza upon arrival. A census was taken and LPA Perry took a census was taken and LPA observed 56 children in care. LPA reviewed and obtained documentation for (1) Type A violation, Title 22 Regulations. Director Cynthia provided LPA Perry with documents from meeting that was held with staff on 3/5/2026 which included transition and name to face procedures. Per facility Director, classrooms will transition in whole groups with two staff, when possible, to help ensure the transition is completed with sweeps and leadership team conducts checks on transitions every 30 minutes to ensure they are completed properly. Additionally, the director has been conducting daily supervision checks every 30 minutes and monitoring of classroom transitions and has provided LPA Perry with form and the end of each day since the citation and will continue for the 30 days since the date of the violation to ensure staff are following required supervision procedures. Declaration was obtained during visit on 3/6/2026. LPA Perry also observed and conducted with facility director the 30-minute checks during inspection. Based on documentation review, LPAs observations and visual documentation received, the deficiency cited on 3/6/2026, has been cleared. LPA Perry provided the facility with a Letter of Deficiency Citations Cleared. This letter must be retained on file for three (3) years and made accessible to the public upon request, according to Division 12, Chapter 1 Regulations. No deficiencies were observed during today’s visit. LIC 9213 (Notice of Site Visit) was issued and must be posted for 30 days. Appeal rights have been provided.
