What the inspector wrote

Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual/random inspection on 01/14/26. LPA arrived to the facility at 8:40AM. LPA met with Syuzanna Jerejyan, director who guided analyst on a tour of the facility. During this inspection licensee, Varduhi Hareyan and Syuzanna Jerejyan, director were also present. This is an infant program which consists of two classrooms; RM #1(0-12 months) and RM #2 (12-24 months). This facility is located on Light on the Corner church property. Facility operation hours are Monday to Friday from 6:30 AM to 6:30 PM. There is a preschool program with Toddler option on the premises with facility number is 198019175. All areas identified on the Facility Sketch were inspected. The following staff were present during this inspection: Room #1 (0-12 months): 6 infants were playing in the classroom with two staff and Room #2: 1 infant with one staff was playing inside the classroom. The following was observed during the tour of facility: LPA observed the facility to be clean, safe, sanitary and in good repair. Furniture and equipment was inspected for age appropriateness and good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings in each classroom. Each infant has a storage bin with his/her name. Hooks are also available to hang coats and sweaters in each classroom. Parents take Linens home each week on Friday or as needed and return it on Monday. Napping equipment (cots) were observed to be stored in a cabinet within the classroom. Age appropriate sinks were inspected for availability and good repair in all restrooms. General sanitation was observed. Per director, cleaning crew clean the facility daily after hours. Availability of indoor drinking water was observed in the classroom. REPORT CONTINUES ON THE NEXT PAGE 1 OF 5 All kitchen areas/food preparation areas in each classroom and food storage areas are kept clean and are free of litter, rubbish and rodents and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. Drinking water is readily available both indoors and outdoors. Children use water bottles indoor and outdoor. The facility was observed to be free of flies, other insects and rodents. LPA observed a microwave, a fridge and bottle warmers in each classroom. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Infant outdoor playarea is physically separated from the other components at this facility. No pets, fire arms or weapons, or bodies of water on the premises. INFANT PROGRAM (in addition to general compliance) Infant changing tables have a padded surface no less than one inch thick and are covered with washable vinyl or plastic and have raised sides at least three inches high. Soiled diapers are sealed in plastic bags and dispose of in a trash can that is inaccessible to infants. Toys were observed to be clean and safe. Infant napping equipment meets the requirements of 101439.1 (a) - (f). Napping equipment and bedding was inspected for good condition, appropriate storage, and cleanliness. Cribs and cots are used for napping and for younger infants are placed in a room separate from the activity area. Infants able to pull themselves up, sleep on mats. Cribs used by younger infants. Mattress is at its lowest position and side rail cannot be lowered. Mattress is covered with vinyl or similar moisture resistant material. Bedding is replaced when wet or soiled or when being occupied by another infant. Parents take linens home on Friday and returned on Monday. Indoor activity space for this infant program is physically separate from the other components at this facility, and meets the requirements of 101438.3 (a) - (e). The facility is in compliance with the staff-infant ratios. A fully qualified teacher is directly supervising no more than 12 infants. Each aide is responsible for the direct care and supervision of a group of no more than four infants. REPORT CONTINUES ON THE NEXT PAGE 2 OF 5 Individual Feeding Plans are on file for each infant that meets the requirements of 101427 (b)(1) - (5). Infant Needs and Services Plan are on file for each infant that meets the requirements of 101419.2 (a)-(b) and 101419.3 (a). Teachers present have proof of the required infant units on file and meet the qualification requirements of 101416.2 (a), (b), (c) and (g). FACILITY RECORDS Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Children's roster was reviewed and is current. Per director, currently there are 20 infants enrolled. LPA observed sign in/out sheet was placed inside by the entrance door. During this inspection 12 children were present and all 12 children were singed in. Disaster drill log was available, last drill was conducted on 12/18/25. Six Children\342\200\231s Records were reviewed for completeness; Inspection of required forms was made and documented on the LIC 857. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. LPA also reviewed 4 staff records. The review of 4 Staff records was documented on the LIC 859. Staff present had proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present had proof against TB, measles, pertussis, and influenza. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. Menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. Infants 0-12 and 12-24 months bring their own food from home. The facility provides emergency AM and PM snack. According to director, medication is only administered to a child when accompanied with a doctor's note and is stored in the child\342\200\231s classroom. The facility was observed to be equipped with an isolation area for any child who becomes ill and it is located in the office. The ill child will use the staff restroom which is located in the second floor during isolation where there is a changing table which is disinfected immediately after use per director. Medication: Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA discussed the safe sleep regulations with director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov . Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. REPORT CONTINUES ON THE NEXT PAGE 4 OF 5 For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed on 03/08/23 in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today.
