What the inspector wrote

An unannounced Required Annual site inspection was conducted by Licensing Program Analyst (LPA) Bardo Baluyot on this day. Upon arrival, LPA conducted a COVID risk self-assessment and met with Site Director, Kelly Quach. This facility is located on the property of Baldwin Elementary School. Access can be gained from the main office located on Almansor St. Hours of operation are Monday, Tuesday, Wednesday, Friday 1:30-6pm and Thursday 12:50-6pm. Upon arrival, LPA observed 2 staff with 21 children. LPA found that facility is operating within ratio. The facility consists of 1 portable (#59) and a playground. The areas utilized by the program are exclusive to the facility and not shared during their hours of operation. LPA inspected all areas of the facility used by children. Furniture and equipment were inspected for age appropriateness and its condition. Telephone service, heating, lighting and ventilation were evaluated. Storage space in form of cubbies and hanging hooks for children's belongings was inspected. Children's restrooms are located outside across from the portable. The condition and age appropriateness of children\342\200\231s sinks and toilets were inspected. LPA evaluated general sanitation and cleanliness of facility. Availability of indoor drinking water was observed in form of a water jug and disposable cups. LPA and director were not able to verify the presence of a carbon monoxide detector to be tested. LPA advised director to follow up with the principal or education coordinator for having one installed in the facility. First aid supplies and medication are stored in a locked cabinet in the classroom. Outdoor playground and equipment were inspected for safety, cushioning material, condition and age appropriateness. The playground is properly fenced. LPA did not observe a source of shade at the time of inspection. The tables and benches available for rest are not covered by any source of shade. Play area was inspected for hazards and inaccessibility to bodies of water. Outdoor drinking water is provided in form of water fountains which were functional at time of inspection. The facility provides an afternoon snack through the Alhambra Unified School District food program which follows all federal guidelines. Snack menu was observed posted in the facility. Required postings were observed on a parent board. Director's First Aid/CPR certifications were reviewed and expire 03/16/2024. LPA reviewed sign in/outs sheets and children's roster. The last disaster drill conducted was on 12/9/2022 and are being documented every month. Medical Services (IMS) policy was discussed. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Files are kept in Options For Learning main office located at: 885 Village Oaks Dr. Ste. #113, Covina CA 91724. No deficiencies cited during today's visit. Exit interview conducted and report was reviewed with the Site Director, Kelly Quach. A notice of site visit was given and must remain posted for 30 days.
