What the inspector wrote

On 12/17/24 Licensing Program Analysts (LPAs) Alicia Mooberry and Joshua Ortega conducted an unannounced Case Management inspection in Spanish, as requirement of the three (3) years probation beginning on 10/07/21. LPAs met with licensee Sandra Benitez and explained the purpose of inspection. Upon arrival, LPAs observed eleven (11) children in the outdoor play area supervised by 2 Staff. Per licensee and facility roster, there are 14 children currently enrolled in in the facility. Individuals residing in the home were discussed and noted. Hours of operation are Mon-Fri 6:00am-5:00pm. Off limit areas including the upstairs level, kitchen and the backyard of home are made inaccessible by safety gates. For the duration of the probationary period from 10/7/21-10/7/24, the licensee has maintained compliance with department regulations and Health and safety regulations. LPA discussed the exclusion of Erick Chajon from the facility pursuant to the Default Decision and Order issued by the Department on December 10, 2020. LPA verified with licensee that the excluded individual is no longer living in the home. LPA did not observe Erick Chajon in the home on this date. LPA confirmed by department records that Erick Chajon is not associated to the facility license. Licensee discussed reporting requirements with licensee. Licensee understands to maintain children's files for three years.
