What the inspector wrote

On 2/4/25, at 11:15PM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced Case Management Visit to this facility. There were 14 children with 2 staff observed present. During a cite visit conducted on 4/30/25, LPA Katrdzhyan discovered that facility staff failed to report an incident to Community Care Licensing (CCL), involving Child 1. The incident occurred during the week of 4/15/25, when Child 1 jumped onto a toy, which later resulted in bruising to the left arm. Licensee / Rubina Isahakyan acknowledged that the incident involving Child 1 was not reported to CCL, as required under Title 22 Regulations Section 102416.2 - Reporting Requirements. Licensee understands the importance of reporting requirements and will submit an Unusual Incident/Injury Report to CCL by no later than the end of business day on 5/14/25, regarding the incident involving Child 1. An Advisory Note - Technical Violation was issued during today's visit stating that Title 22 Regulations Section 102416.2 was discussed with Licensee and an In-service Training will be conducted and copies of the training along with signatures of all attendees will be submitted to CCL by the end of business day on 5/16/25. The Notice of Site Visit was provided and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Rubina Isahakyan and Appeals Rights provided.
