On 03/12/25, at 1:20 PM , Licensing Program Analyst(LPA) Shushanik Safaryan conducted an unannounced Case Management inspection to the above facility to ensure Type B deficiency cited on 01/31/2025 has been cleared. Upon arrival LPA met with Naira Aghavelyan , licensee who guided analyst on a tour of the facility. During this inspection, LPA observed 3 napping children . During the visit LPA toured the facility ,reviewed children files and observed missing immunization's obtained , observed required 2A10BC fire extinguisher in the kitchen with the purchase date 02/09/2025. During the visit, LPA cleared deficiency cited on 01/31/2025 and provided a copy of the Licensing Report to Licensee and issued POC clearance letter. The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
