What the inspector wrote

On 08/22/2025, Licensing Program Analyst (LPA) Kruz Long conducted an unannounced case management inspection. LPA met with Monique Mabunay, Program Director and explained the purpose of the visit. There are 39 children with 8 staff members present during today's visit. The purpose of today’s visit is to follow up on an incident that occurred on 08/20/2025 and was reported to the department on 08/21/2025. The self reported incident is regarding supervision. During the course of the investigation, LPA interviewed Staff #1 (S1) to Staff #3 (S3), obtained a copy of the children roster and training documents. According to interviews with S1 to S3, it was disclosed that on 08/20/2025 parents were picking up children from Room 2. After leaving a parent did not close the classroom door. Staff #2 (S2) conducted a head count and notice that C1 was not in Room 2. Staff in Room 1 was made aware and Staff #3 (S3) went to look for C1 and found C1 in Room 3 by themselves. S3 then guided C1 back to Room 2. C1 was missing for approximately 5 minutes. Documents reviewed indicate additional training was provided to Staff regarding supervision. Based on interviews conducted and records review, the deficiency was cited in accordance with the Title 22 of the California Code of Regulations and Health & Safety Codes. Please see LIC809D for documentation of deficiency. Upon receipt of the licensing report. Licensee shall post report documenting Type A citation along with Notice of Site Visit Form in an area accessible for review for 30 days or a civil penalty of $100 will be assessed. Licensee shall also provide a copy of the report documenting Type A citation and any report resulting from a Non-Compliance Conference to all parents of currently enrolled children along with form LIC 9224 (Acknowledgement of Receipt of Licensing Report) for signature. Completed form shall be placed in child's file. Licensee shall also provide report and form to parents of newly enrolled children for the next 12 months. An exit Interview was conducted, a copy of this report and appeal rights along with Notice of Site visit was provided to the Program Director.

This requirement is not met as evidence by: According to interviews with S1 to S3, it was disclosed that on 08/20/2025 C1 was missing from room 2 for approximately 5 minutes and was found in room 3.
