What the inspector wrote

Licensing Program Analyst (LPA) Stephanie Li conducted an unannounced Case Management inspection due to an incident that occurred on 6/24/2025 and was reported to the Department on 6/25/25. A risk assessment was conducted. LPA met with Director Kara Lewis, to whom the reason for the visit was explained. Director guided LPA on a tour of the facility. There were 28 children and 6 staff. The facility was observed to be operating within the license capacity limitations. On 6/25/2025, an unusual incident report was made to the Department regarding an incident that resulted in a child getting injured and receiving 5 stitches. A 2 year old was running in the classroom, tripped over his own 2 feet and fell into a shelf and sustained a gash on his forehead. The teacher was addressing the running child to slow down and he tripped right after she reminded him. Director immediately assisted with first aid, parents were contacted and picked up child. Child was brought to the hospital and needed 5 stitches. School was closed for 4 th of July week and opened on Monday 7/7/25. Child returned to school Monday 7/7/25. During todays visit, LPA interviewed Director and 1 teacher regarding the incident, inspected classroom furniture, and child\342\200\231s medical docs, obtained doctors note clearing child to return to school with no restrictions. LPA observed the classroom furniture placement to be safe. Cubicle that child landed on is a 4 cubbie shelf from target or Ikea with sharp edges. LPA advised director to bumper cushion the edges to soften impact in case of future falls. page 1 of 2 Based on information obtained during this visit, no follow up is necessary regarding the incident reported. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Director Kara Lewis .
