What the inspector wrote
An unannounced, in-person Annual/Required-1 year inspection was conducted on this date by Licensing Program Analysts (LPA) Emiko Bell.
At 09:30 am, LPA was greeted and let into the residence by licensee's spouse/assistant, Edgar Pulley, to whom the reason for the inspection was announced. Mr. Pulley assisted LPA the duration of the inspection, as licensee is Spanish-only speaking.
At 09:40 am, Mr. Pulley began guiding LPA on a tour of the residence. The tour was completed at 10:50 am.
Census
:
Staff-child ratio was met There were two adults and 11 children present (one is of age to not count in the census). Both staff are cleared and associated.
Licensee's days and hours of operation are Monday-Friday, 06:00 am-06:00 pm. This is a single-story, single family residence with three bedrooms and two bathrooms. All areas identified on the facility sketches were inspected in the following order: (indoors): the living room, the laundry room, bedrooms #1 & #2, bathroom #1, the master bedroom and bathroom, the kitchen, (outdoors): the backyard and then (indoors): the garage.
The following areas have been designated as off-limits; however, these were inspected by LPA today

Physical Plant:
The residence was inspected for safety, comfort, cleanliness, telephone service (licensee and her spouse each have a cell phone); heating and ventilation (there is a ceiling fan in bedroom #2 and there is central heating and air-conditioning); inaccessibility to poisons, detergents, cleaning compounds (kept in the laundry room and under the kitchen sink, both of which are rendered inaccessible by childproof latches); medicine (stored on a high shelf in one of the kitchen cabinets) and other hazardous items that can pose a danger to children.
Toys and napping equipment:
There are age-appropriate toys and napping equipment on the premises. Licensee has one crib, one play yard
,
and tri-fold mats
.
The children nap in the living room.
Parent Board:
Licensee's Parent Board is located on the wall to the left of the entryway, past the hallway.
Pets:
Licensee has two dogs, which are kept in a fenced-off area of the backyard.
Fire safety:
Licensee has a fire extinguisher, size 2-A:10-B:C,
which is mounted on a living room wall; it was last serviced 06/22.

icensee keeps an emergency drill log; the last fire drill was run 04/19/22 and the last earthquake drill was run 05/17/22. There are five smoke detectors; all were tested and are operable. (The smoke detector in the living room is a combination smoke/carbon monoxide detector.)
Transportation:
Though
Mr. Pulley transports the daycare children, his vehicle was not inspected during today's visit. Per licensee's spouse/assistant, he has one booster and two regular car seats.

Firearms: Mr. Pulley
stated that there are currently no firearms or weapons on the premises.
Incidental Medical Services:
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Backyard:
The backyard is fenced on one side by a wooden fence, on one side by a brick wall and on one side by a brick wall with a chain-link fence on top of it..
Paperwork:
Licensee has a Child Care Facility Roster; it is current and complete. Licensee's Pediatric First Aid/CPR was issued by Medic First Aid, which is EMSA-certified; it expires 11/30/22. Mr. Pulley's Pediatric First aid/CPR was also issued by Medic First and expires 03/05/24. Both licensee and her spouse/assistant completed Mandated Reporter Training on 01/14/22 through Child Care Providers United. Both licensee and Mr. Pulley have verification of MMR and TDAP immunizations and TB clearance.
Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, LIC 9227- Infant sleep form (0-12 months, and documentation of 15-minute Infant Sleep Check (0-24 months)

Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC-501: Personnel Record, LIC 508- Criminal Record Statement, LIC 9052- Employee Rights, Proof of immunization's against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate.
Based on the LPA's observations and records review, no deficiencies will be cited today, 06/02/22.

Licensee's spouse/assistant was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
LPA discussed the safe sleep regulations with licensee's spouse/assistant and discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPA also informed licensee of the importance of checking for

recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
A notice of site visit was given and must remain posted for 30 days.
