What Happened
Case management incident investigation following child's right femur fracture on 10/8/2025 during PM recess. Investigation found facility supervision plan was not followed, yellow slide area was left unsupervised, and staff assigned to area was not positioned to observe slide per guidelines. Type A citation issued indicating immediate risk to health and safety.
Observations
- Child #1 sustained right femur fracture on 10/8/2025 during PM recess when child #2 slid down yellow slide and struck child #1 seated at bottom of slide
- Staff #1 assigned to supervise yellow slide on Playground 4 was not positioned to observe the area per supervision guidelines
- Playground Supervision Checklist not completed daily by Director - only at least one or two days per week
- Completed Playground Supervision Checklist not available for 10/8/2025 incident date
- Inconsistent staff statements regarding location and observations during incident
- Video camera (Camera 1) covering Playground 4 was offline at time of incident - cameras go offline due to outdoor weather/moisture
- Discrepancy between weekly headcount sheet (54 children) and actual staff records timecards showing 60 children and 7 staff present on 10/8/2025 at 4:30 pm
