What the inspector wrote

On 12/10/25 Licensing Program Anaylst (LPA) Tyler Reyes conducted an Unannounced Case Management Inspection and met with Licensee ALVAREZ LEIVA. LPA disclosed the purpose of the inspection and was granted entry into the facility and was provided with a guided tour. The purpose of the visit was to follow up on information received regarding dogs at the facility. It was reported that the dogs had gotten loose and had an altercation with other dogs being walked. The visit was to verify that the Licensee is ensuring the dogs do not pose a risk to children in care. Per Licensee the facility has (3) pets. LPA observed the (2) pets located in a separate area of the backyard secured by a metal gate that swings away and is self latching. (1) pet was located in the off-limit room. Licensee stated pets are kept separated during operating hours. LPA reminded the Licensee to ensure constant supervision and maintain safe practices at all times. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days consecutive days . Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview conducted and a copy of the report and appeal rights were provided to the Licensee ALVAREZ LEIVA. No deficiencies cited.
