What the inspector wrote

On 3/11/26 Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced case management inspection to follow up on an incident that was reported to the Department on 3/10/26. Upon arrival, LPA met with Facility Representative Vanessa Ramos. LPA informed of purpose for visit and was provided a tour of the facility inside and outside. LPA observed 11 children and 3 staff in the playing on playground. Early Learning Center Managers, Blanca Litzinger and Monica Alvarado arrived during inspection. The facility reported that on 3/10/26 at approximately 8:50am, Child #1 (C1), finished breakfast and solely transitioned to the carpet to work on floor puzzle, C1 was spinning and and fell hitting a chair causing an injury to C1's mouth. During this visit, LPA conducted interviews with staff, children, authorized representative and inspected classroom where incident happened. Interviews conducted confirmed the details of the incident report. Per Staff 1, who was present and witnessed the incident, C1 was immediately provided 1st aid. C1's parent was called at 9:00am and picked up child by 9:59am and took child to the dentist. Per Authorized representative, C1 will stay home for a short time to receive care since there are dietary restrictions. LPA observed that the floor in the room in clean and in good repair. Child staff ratios were observed to be 2 staff and 9 children and in compliance with Title 22 regulations. Per facility representative, the chair that was at the edge of the carpet for teacher's use has been removed when teacher is not using it as a precautionary measure. Based on Interviews, observations and record reviews there are no deficiencies cited as a result of this inspection. A notice of site visit was given and must remain posted for 30 days.
